Installation:
Apache, php5, mysql client, tcpdf class
1. Copy inc/config.inc.php.example to inc/config.inc.php and edit this file according to your db setup
2. Get tcpdf class (http://www.tcpdf.org/) and make files accessible in inc/tcpdf
3. edit sql queries and label layout in ./tc_label.php according to your needs
4. secure this directory

Point your browser to the script URL..
